Dr. Callegari is an obstetrician gynecologist and health services researcher. Her work focuses on developing and testing interventions to improve provider-patient communication, patient-centered care, and equity in reproductive health. Funded by an HSR&D Career Development Award, she developed and piloted a web-based tool to facilitate shared decision making conversations about pregnancy and contraceptive decisions and, ultimately, to support reproductive autonomy and improve reproductive health outcomes among women Veterans. She is now conducting a multicenter randomized controlled trial of the tool in VA primary care settings.

She also studies mechanisms of racial disparities in gynecologic outcomes in the VA to inform interventions to promote equity in VA gynecologic and surgical care. She attended Harvard University for medical school and the University of California, San Francisco for her residency in Obstetrics & Gynecology. After working in clinical practice for several years, she completed an MPH in epidemiology at the University of Washington and a VA postdoctoral fellowship in health services research. She is currently Associate Professor of Obstetrics & Gynecology and Adjunct Associate Professor of Health Services at the University of Washington and a Core Investigator at the VA HSR&D Seattle-Denver Center of Innovation for Veteran-Centered and Value-Driven Care.

MyPath: A Patient-Centered Web-Based Intervention to Improve Reproductive Planning for Women Veterans

Women Veterans who use VA healthcare face elevated risks of poor pregnancy outcomes compared to civilians due to higher rates of chronic medical and mental health conditions. Proactive planning and optimization of health before pregnancy can mitigate these risks; however, nearly 40% of pregnancies among Veterans are unplanned. Although national guidelines recommend routine assessment of women’s childbearing goals followed by tailored contraceptive and preconception counseling in primary care, these reproductive planning services are delivered infrequently in VA.
This study is a multicenter pragmatic randomized controlled trial of “MyPath,” a novel web-based patient-facing decision support tool. MyPath is designed to be used by women Veterans before clinic visits to increase patient-centered reproductive planning discussions and facilitate high-quality decisions. If effective, this innovative, scalable intervention will improve the delivery of guideline-recommended services as well as reproductive health outcomes in Veterans nationally.

Understanding Racial/Ethnic Disparities in Uterine Fibroid Outcomes among Women Veterans: A mixed-methods study

Uterine fibroids (UF) are a widespread and costly gynecologic condition that disproportionately affects Black women concerning incidence, severity, treatment, and outcomes. Recent data suggest substantial Black/White disparities in UF exist within VA. As one-third of women Veterans using VA health care are Black, and this proportion is increasing, understanding the drivers of these disparities within the VA is of critical importance.
This study will use mixed methods to investigate mechanisms underlying Black/White disparities in UF-related outcomes in VA, including modifiable differences by race in pathways to treatment and interactions with the healthcare system.
This work addresses multiple HSR&D priority areas, including women’s health, health equity, and healthcare access. It will make a substantial impact by informing the development of patient-centered interventions to eliminate disparities. The steps that follow will include partnering with VA women’s health and health equity leaders to design programs and policies that can accelerate progress toward quality and equity in gynecology care in VA.
