Research Interests

Karin (“Kari”) Nelson, MD, MSHS, is a general internist and health services researcher at the VA Puget Sound Health Care System. She is a Professor of Medicine in the University of Washington (UW) School of Medicine, Adjunct Professor in the UW School of Public Health and core investigator at the Seattle Health Services Research & Development (HSR&D) Center of Innovation (COIN).

Dr. Nelson is the director of the Primary Care Analytics Team (PCAT) for the VHA’s Office of Primary Care. As an investigator for the PACT national evaluation, she won the 2015 VHA HSR&D’s Best Research Paper of the Year Award for Nelson, et al. “Implementation of the Patient Centered Medical Home (PCMH) in the Veterans Health Administration (VHA)” published in JAMA Internal Medicine. Her primary research focus is in improving disease self-management, focusing on peer support interventions. She actively mentors many junior faculty and is the former Co-Director for the MD fellowship program in the Seattle HSR&D COIN. She is a primary care physician at the VA Puget Sound. Dr. Nelson received her medical degree from the University of Minnesota, and her Masters in Health Services from UCLA.

Telehealth-delivered peer support to improve quality of life among Veterans with multimorbidity

Co-PI with Kristen Gray
Multimorbidity is common among Veterans and results in poor health-related quality of life (HRQoL). It contributes to high health care utilization and costs. Patients with multimorbidity need to engage in self-management for many of their chronic conditions. Patients with multimorbidity experience significant barriers to self-management. Due to the limitations of traditional primary care visits, they need additional support to engage in self-management optimally. Peer support delivered by trained Veteran peer health coaches is well-suited to fill this need. Limited research has evaluated the effectiveness of peer support in multimorbidity.
The proposed study evaluates whether a telehealth-delivered peer health coaching intervention improves HRQoL compared to usual VA care among high-risk patients with multimorbidity. It also aims to understand barriers and facilitators to implement the program in VA if it is effective.
This research may enhance VA’s ability to provide services to Veterans with multimorbidity and improve health outcomes.

Developing a New Method to Improve Primary Care Workforce Management

The Department of Veterans Affairs (VA) provides primary care to over 5.7 million Veterans. In 2010, VA primary care moved to a medical model called the Patient Aligned Care Team (PACT). In PACT, Veterans are assigned or “empaneled” to a primary care provider and team. Current VA panel size estimates are based on historical means.
A recent HSR&D Evidence Synthesis Program (ESP) report calls for more research to manage the primary care workforce of 7,995 VHA primary care physicians and providers.
The proposed project adopts a previous method to calculate supply and demand for primary care services by using time estimates to describe overflow, where demand would exceed supply for primary care services.
The results of this study have a high probability of immediate impact on the VA by informing primary care workforce management, in addition to providing evidence that longitudinal patient histories are useful in estimating time and workload for primary care panels.

Vet-COACH (Veteran peer Coaches Optimizing and Advancing Cardiac Health)

The Vet-COACH (Veteran Peer Coaches Optimizing and Advancing Cardiac Health) study will test the effectiveness of a peer health coach intervention that provides health education and linkages to community and clinic-based resources to improve the control of cardiovascular disease (CVD) risk factors.
Given that the majority of Veterans have one or more CV risks, cost-effective mechanisms are needed to manage the burden of CVD risk.
Veteran peer health coaches may be one such mechanism; however, models of this type of intervention have not been tested within VHA.
Utilizing Veteran health peer coaches may improve the VA’s ability to provide community outreach and in-home services for Veterans with poor CVD risk factor control and ultimately link Veterans to the VA health care system to improve health outcomes and decrease health care costs and utilization.

Description: Primary Care Analytics Team (PCAT) provides analytic resources for the Office of Primary Care for program evaluation, program planning, and innovation development. PCAT mission is to execute analysis related to VHA Primary Care and respond to the needs of Office of Patient Care (OPC)’s priorities (Fidelity to the PACT model, high risk patients, virtual care). This presentation will provide an overview of PCAT work and highlight select projects.
