Implementation & Adaptation Guide for
Care Coordination Interventions


This web guide is intended for anyone who is planning to implement a new practice into any healthcare setting. It provides tools and steps to implement and adapt an evidence-based practice/program/intervention – we use these terms interchangeably. It will work for multiple levels (for example, hospital transitions, community care, primary care teams) within different settings. We use examples from our previous work with implementing care coordination interventions, but you can use these steps to adapt and implement any evidence-based practice into your environment.

This guide follows the steps outlined in the Implementation Roadmap (email us if you would like a copy of the Implementation Roadmap), developed by the US Department of Veterans Affairs Quality Enhancement Research Initiative (QUERI). The QUERI Implementation Roadmap facilitates uptake of an intervention or evidence-based practice through three phases:

We illustrate some of these concepts using examples from our projects, Community Hospital Transitions Program (CHTP) and Advanced Care Coordination (ACC). These examples will use the exclamation point and be offset as this is.
